Ear Pinning (Setback Otoplasty)
The most common technique. An incision behind the ear exposes the cartilage; permanent sutures fold and anchor the antihelical fold, bringing the ear closer to the head. The angle between the ear and skull is reduced from the typical prominent 35–45° to a natural 15–20°. No cartilage is removed.

Ear Reduction
For ears that are proportionally large (macrotia) rather than simply prominent, a segment of cartilage and skin is removed to reduce the overall ear size. Incisions are placed within the natural folds of the ear. Results are permanent with no external visible scarring.

Combined Reshaping
Addresses both prominent protrusion and irregular cartilage shaping (absent antihelical fold, deep conchal bowl) in the same procedure. Most frequently required when one ear differs significantly from the other in both angle and form.

Recovery Timeline
Stage 1
Day 1 — Consultation & Marking
Photographs taken from front, side and rear. Surgeon measures ear protrusion angle and assesses cartilage flexibility. The desired new ear position is marked and shown to the patient using a mirror. Suitable age for children confirmed (typically 6+ years when ear growth is 90% complete).
Stage 2
Day 2 — Surgery
Local anaesthesia for adults and teenagers; general anaesthesia for young children. Duration 1–2 hours. A bulky protective head bandage is applied immediately after surgery to hold the ears in their new position. Discharged the same day.
Stage 3
Day 5–7 — Bandage Removal
The head bandage is removed at the clinic. A lighter elastic headband is worn for a further 3–4 weeks — primarily at night. Sutures behind the ear are removed at this appointment.
Stage 4
Week 2 — Return to Normal Activity
Children return to school and adults to work. Hair can be washed gently. Contact sports and activities that risk impact to the ears are avoided for 6 weeks.
Stage 5
Month 1 — Final Position Confirmed
Any residual swelling behind the ear has resolved. The ears sit in their permanent position. Scars behind the ear are pale and hidden within the natural groove — imperceptible in all normal hairstyles.
