How Many Grafts Do I Need? Norwood Scale Explained (2026)

Most hair transplant patients need between 2,000 and 4,500 grafts, and your exact number is set by three things: your Norwood stage, the size of the area to be covered in cm², and your donor capacity. As a working guide: Norwood 2 needs 1,000–1,500 grafts, Norwood 3 needs 1,800–2,500, Norwood 4 needs 3,000–4,000, Norwood 5 needs 3,500–4,500, and Norwood 6–7 needs 4,500–8,000 grafts, usually split across two sessions. The Hamilton–Norwood scale is the international standard for classifying male pattern hair loss. It has seven stages, and almost every surgeon, clinic and forum worldwide uses it as a common language.

Stage 1 — no meaningful recession. Juvenile hairline intact.

Stage 2 — slight recession at the temples. Usually not a surgical candidate yet.

Stage 3 — the first stage medically classified as balding. Deep, symmetrical M-shaped temporal recession.

Stage 3 Vertex — hairline of a Stage 3 plus visible thinning at the crown.

Stage 4 — wider frontal recession plus a defined bald patch at the crown, separated by a band of hair across the mid-scalp.

Stage 5 — the same pattern, but the separating band is narrower and thinner.

Stage 6 — the bridge is gone; front and crown merge into one continuous bald area.

Stage 7 — only a horseshoe of hair remains at the sides and back.

Staging yourself from a mirror is unreliable — most patients under-estimate crown loss by a full stage. Photos from the top and back are what a surgeon actually needs.

The 2026 Istanbul market sits at roughly €0.65–€1.10 per graft, though serious clinics price by package rather than per graft — because the real cost drivers are surgeon involvement, graft handling and aftercare, not arithmetic. One graft is one follicular unit, and a follicular unit contains 1 to 4 hairs — on average 2.1–2.4. So 3,000 grafts is roughly 6,300–7,200 individual hairs.

This is the single most common way patients are misled. A clinic advertising “7,000 for €1,800” is almost always quoting hairs, not grafts — which is around 3,000 grafts. Always ask, in writing: how many grafts, not how many hairs?

Surgeons measure the bald zone in cm² and multiply by a target density. A natural, front-facing hairline needs 35–45 grafts/cm²; the crown is usually restored at 25–35 grafts/cm². A 60 cm² frontal zone at 40 grafts/cm² = 2,400 grafts. That is the actual maths behind your quote.

Your safe lifetime donor supply from the occipital and temporal zones is typically 6,000–8,000 grafts, at a native density of 60–100 follicular units per cm². Beard and chest hair can supplement high-Norwood cases. A weak donor caps your plan regardless of how much coverage you want — and over-harvesting leaves permanent visible thinning at the back.

Thick, wavy, light-coloured hair covers far more scalp per graft than fine, dark, straight hair on pale skin. Two Norwood 4 patients can legitimately need 3,000 and 4,200 grafts respectively.

Under 30 with rapid loss, the plan must budget for future loss. A dense, low, teenage hairline placed at 25 can look isolated at 40. We plan a mature hairline and recommend medical therapy (finasteride/minoxidil, where medically appropriate) to protect the native hair behind it.

If your donor is limited, prioritise the frontal third. It frames the face, it is what people see in conversation and in photos, and it delivers the highest visual return per graft. The crown is a spiral whorl that scatters light and demands more grafts for the same perceived density — it is the least efficient area to spend a limited donor on.

No responsible clinic exceeds roughly 4,500–5,000 grafts in a single session. Beyond that, grafts spend too long outside the body, survival rates fall, and the donor area is over-harvested for a one-off result. Is a higher graft number always better? No. Placing more grafts than your donor can sustainably give, or packing them too densely, reduces survival and damages the donor area permanently. The correct number is the number that fits your donor and your long-term pattern.

How many grafts do I need for a receding hairline only? Typically 1,500–2,500 grafts for the hairline and frontal third, in a single session, with results visible from month 6 and final at month 12.

Can I have 3,000 grafts if my donor is weak? Sometimes — with beard grafts added, or by reducing target density and covering a smaller priority zone. Our medical team will tell you honestly if the plan you want is not achievable in one pass.

Do quoted graft counts include the crown? At Firma Clinic, yes — the written plan lists grafts per zone. Elsewhere, always confirm; “3,000 grafts” for a Norwood 5 covering only the front is a very different result from full coverage.

One clinic quoted 3,000 grafts, another 5,000. Who is right? Ask both for grafts per cm² and per zone. Inflated counts are a known sales tactic, and counts are rarely audited during surgery. A clinic that shows you the measurement logic is the one to trust.
